This form is for use only by NJ EMS Agencies expressing interest in assisting with coverage of FIFA World Cup 2026™ activities.  By completing this form, you are only expressing interest in the assets listed.  This is not a commitment.  A Professional Services Agreement and a Certificate of Insurance will be required if services are accepted.  Any reimbursement, if available, will be limited to earned overtime only.   There will be no reimbursement for equipment, base rate, fringe, mileage, etc.   By assisting, the agency retains all rights and responsibilities. We will coordinate through the FIFA World Cup EMS Operations Center ([email protected]) and your assigned Task Force Leader/Crew Boss.
