The survey link below will be utilized to collect agency information and the quantity of bags to be shipped. Please submit this form by the 15th of the month to receive bags for the following month. Survey Link: https://njdoh.iad1.qualtrics.com/jfe/form/SV_8fbiJzX4M3xHW0C Narcan Leave Behind Documentation for EMS Agencies: For EMS agencies only, when naloxone is left behind with a patient, the EMS provider MUST document that in the electronic patient care report (ePCR) under the medication section with the route equaling “left behind/not administered.” (e.g. Medication = Naloxone; Dose= 4mg; Route = “left behind/not administered”) If this choice is unavailable in your ePCR Software, please get in touch with your vendor immediately. It is critical that all providers document this correctly within the chart so appropriate supplies can be ordered. Please ensure all of your crews are aware of the documentation requirements.
